Adaptive designstrategier i medicinske forsøg – en evaluering.
Den seneste tid har brugen af adaptive designstrategier i medicinsk analyse og vækst, der hovedsagelig er baseret på periodiseret information, vist sig at være meget omtalt, der kan tilskrives dens fleksibilitet og effektivitet. Primært baseret på anvendte variationer kan adaptive design kategoriseres i tre klasser: potentielle, samtidige (advert hoc) og retrospektive adaptive design.
Et adaptivt design tillader ændringer foretaget forsøg og / eller statistiske procedurer for igangværende medicinske forsøg. Ikke desto mindre er det en bekymring, at de nøjagtige berørte personer indbyggere efter variationerne kan afvige fra det oprindelige mål berørte person indbyggere og følgelig ikke den generelle type I fejl (for at fejlagtigt erklære effektivitet for et infektionsmiddel) gebyr ikke vil blive styret.
Samt vigtigste variationer af forsøg og / eller statistiske procedurer for igangværende forsøg kan ende i en helt anden prøve, som ikke er i stand til at behandle de videnskabelige / medicinske spørgsmål, som forsøget har til hensigt at besvare. På denne artikel gennemgås et antal generelt tænkte på adaptive design i medicinske forsøg. Virkningerne af annonce hoc-variationer (protokolændringer), udfordringer i efter design (potentielle) variationer og hindringer for retrospektive variationer er beskrevet.

Metoder til anvendelse af adaptivt design i medicinsk vækst af usædvanlige sygdomme er nævnt. Nogle eksempler på vækst af velcade antaget til et antal myelomer og ikke-Hodgkins lymfom er givet. Der nævnes desuden fornuftige punkter, der generelt kan opstå ved implementering af adaptive designstrategier i medicinske forsøg.

Miljø- og erhvervsmæssige årsager til de fleste kræftformer: nyt bevis 2005-2007.
Hvad vil vi i øjeblikket vide om erhvervsmæssige og miljømæssige årsager til de fleste kræftformer? Fra 2007 anerkendte det verdensomspændende firma til analyse af de fleste kræftformer (IARC) 415 identificerede eller mistanke om kræftfremkaldende stoffer. De fleste kræftformer opstår ved hjælp af et særligt sofistikeret internet af en række årsager, og vi ved sandsynligvis på ingen måde den samlede variation af mæglere eller blandinger af mæglere.
Vi ved, at det at forhindre sygdommen at stoppe reklame for bestemte kræftfremkaldende personer. Fald i de fleste kræfttilfælde – sådan som faldet i mandlig lunge de fleste kræftformer omstændigheder fra rabatten i tobaksrygning eller faldet i blæren de fleste kræftformer blandt kohorter af farvestofansatte fra eliminering af reklame til særlige duftende aminer – giver bevis for, at de fleste kræftformer stoppes er muligt, når vi handler efter det, vi alle ved.
Selvom den generelle aldersjusterede flest kræftformer i USA blandt hver kvinder og mænd er faldet inden for det sidste årti, er afgiften for en række kræftformer stigende; et par af dem er knyttet til miljø- og erhvervsmæssige eksponeringer.
Denne rapport fortæller om det nyeste epidemiologiske bevis, der forbinder erhvervs- og miljøeksponeringer med de fleste kræftformer. Peer-reviewed videnskabelig forskning, der blev afsløret fra januar 2005 til juni 2007, var blevet gennemgået, som supplement til vores state-of-the-evidensrapport, der blev afsløret i september 2005.
Uanset svagheder i en bestemt persons forskning overvejer vi beviset, der forbinder den hævede fare for en antal sorter af de fleste kræftformer med særlig eksponering, der er væsentligt styrket af de nyeste publikationer, blandt dem husker de fleste kræftformer fra reklame til ikke-ioniserende stråling, markant radiofrekvensfelt udsendt af mobiltelefoner; bryst de fleste kræftformer fra reklame til pesticidet dichlorodiphenyltrichlorethan (DDT) tidligere end puberteten; leukæmi fra reklame til 1,3-butadien; lunge flest kræftformer fra reklame til luft luftforurening; ikke-Hodgkins lymfom (NHL) fra reklame til pesticider og opløsningsmidler; og prostata de fleste kræftformer fra reklame til pesticider, polyaromatiske kulbrinter (PAH’er) og metalliske arbejdsvæsker eller mineralolier.
Sammen med de fleste kræftformer i NHL og prostata rådgiver de tidlige resultater fra de landsdækkende institutter for velvære inden for landbrugs-velvære, at en række yderligere kræftformer også kan knyttes til en hel del pesticider.
Vores rapport beskriver desuden kort det toksikologiske bevis, der er forbundet med kræftfremkaldende påvirkning af bestemte kemiske stoffer og mekanismer, som kan være besværlige at gennemgå hos mennesker, især eksponering for bis-fenol A og epigenetiske, transgenerationsresultater.
For at understrege den multifaktorielle, flertrins karakter af de fleste kræftformer, opdaterer vi desuden en teknisk beskrivelse af de fleste kræftformer, der sammenfatter de nuværende data i molekylærbiologi. Vi argumenterer for et helt nyt mest kræftforebyggende paradigme, der hovedsagelig er baseret på en forståelse af, at de fleste kræftformer endelig bringes videre af et antal interagerende elementer noget end et paradigme, der hovedsageligt er baseret på tvivlsomme fraktioner.
Dette nye mest kræftforebyggende paradigme kræver, at vi begrænser reklame til undgåelige miljø- og erhvervsmæssige kræftfremkaldende stoffer sammen med yderligere væsentlige fareelementer som madplan og livsstil. Analyselitteraturen forbundet med miljømæssige og erhvervsmæssige årsager hos de fleste kræftformer stiger konstant, og fremtidige opdateringer skal udføres i skånsomhed med den nyeste organiske forståelse af mekanismerne og nye strategier til at finde ud af eksponeringer i menneskelige befolkninger.

Den verdensomspændende ImMunoGeneTics-database (IMGT) , er et indbygget informationssystem af topkvalitet, der specialiserer sig i immunoglobuliner (IG), T-celle-receptorer (TR) og Main Histocompatibility Advanced (MHC) for human og forskellige hvirveldyr oprettet i 1989 af Laboratoire d’ImmunoGénétique Moléculaire (LIGM) på Université Montpellier II, CNRS, Montpellier, Frankrig.
IMGT tilbyder en typisk indgang til standardiseret information, der omfatter nukleotid- og proteinsekvenser, oligonukleotidprimere, genkort, genetiske polymorfismer, specificiteter, 2D- og 3D-bygninger.
